Can someone help me make a taper plan?

I'm on 4 mg of Ativan.  First off I don't know equivalent dose of Valium would be.  I want to c/o. Some say for every 1mg of Ativan its 5mg ofvalium. While others say it's a 1:10 ratio. 

 

Also how much do I reduce per week roughly? Dose anyone one know of a schedule I could follow.

The equivalent would be 40 mg of Valium. First a cross over would need to be done and then a 5 to 10 percent reduction every two weeks from there. You may want to post on the substitution plan board to find out more about how to cross over.  I personally do not have any first hand experience.  However, I do know that because of Valium's very long half life that it is a good choice to taper from.
