Confused about lorazepam liquid titration taper

Hi all... I've read some conflicting information on this board about crushing ativan pills for liquid titration.

 

I know that ativan is not water or lipid soluble, but we're making a suspension, not a solution, so wouldn't it be better to use milk, which is more viscous, instead of water?

  Milk wouuld be better than water, but neither will make a very reliable suspension.

 

To make a good suspension, use OraPlus/OraSweet.

 

Or use propylene glycol and make a true solution.

You can definitely dissolve it in a little PG and dilute it with water.  I don't know about milk.

 

( I would suggest 2mg PG per 1mg lorazepam)

I don’t understand why you don’t get your doctor to prescribe liquid ativan. You just put your dose in a syringe and swallow.

My psych provider doesn't see the need for liquid ativan.  He thinks I should drop by 0.25 every month, although he has said he doesn't mind me staying on 1mg/day for as long as I want, forever if necessary.  I don't think I can convince him to prescribe liquid ativan.  I will try convincing him again in the future, but I'm holding for a few months right now.

You can easily make your own liquid loarzepam from your tablets,  with propylene glycol and water.

It's me again.

 

if I were to make my own liquid lorazepam suspension (my doctor seeming unwilling to do the concentrate) how would I do it?

 

I need to do the cross over from lorazepam to valium, as I think you know.  I'm at .375 mg lorazepam (or more depending on the accuracy of the pill cuts).  Via the Ashton method chart, I should go down to

.312 lorazepam substituting .625 mg valium (one week)

.250 mg to 1.25 V (easy, because that's half an .05 mg pill and I can do dry cut) (one week)

.188 mg to 1.88 V (one week)

.125 mg (1/4 pill, I can do dry cut) to 2.5 mg V (one week)

.063 mg to 3.13 mg V (one week)

fully convert to 3.75 diazepam then taper

 

This five week crossover may be too long, as I'm experiencing withdrawal symptoms now.

I suppose I could try cutting 1/4 pill into an 1/8th.

Dissolve 5mg lorazepam in 10ml propylene glycol, and add 40ml water.  Then you still have a solution with .1mg=1ml.

 

At your dosage, that's about a 2 week supply. 

 

Propylene is available from Amazon, large pharmacies, and specialty food stores.

No.  Store at room temp, protect from light.
