Help with ativan - valium substitution needed!

How would folks suggest moving from the 0.63 mg lorazepam to Valium? Nervous. Interdose w/d and tolerance w/d have been brutal. Currently I'm dosing 4x day with more in the middle of the day than at the ends.

The Ashton crossover tables seem to work well for most and would be a good start for you.  There is also a different plan by Reg Peart that I am not familiar with, but have seen where others have been happy with it.

Those tables start with higher doses of lorazepam (3 mg minimum). Would I do some division to figure out the equivalents for my starting dose of 0.60 mg? And would I shorten the amount of time?

I'm no expert, but that would be my thought as well.  In the 3mg/30mg table it is done in six steps with 1 week hold each.  So, if you are on .63mg that would be .105mg each week.  Does your lower dose make a difference?  I don't know.  I wouldn't think so though.

I was wondering how you were doing!  I hope you get some good advice, it seems like the valium cross over helps a lot of people
